U.S. Department of Health and Human Services, is a nationwide service
that connects older Americans and their caregivers with information
on senior services. The Eldercare Locator is designed to help elder
adults and their families and caregivers find their way through
the maze of services for seniors by identifying trustworthy local
support resources. The goal is to provide users with the information
and resources they need that will help older persons live independently
and safely in their homes and communities for as long as possible.

A geriatric care manager is a health and human services professional,
such as a gerontologist, nurse, social worker, or psychologist with
a specialized focus on issues related to aging and elder care. Geriatric
care managers work privately with older adults and their families
to create a plan of care that meets the needs of the older adult.
They will meet with you to help you understand your loved one's
needs and to learn what resources and options are available to meet
those needs.
